Parent's or Guardians' Permission & Waiver of Liability & Authorization for Emergency Care

I hereby give my consent for the registrant to participate in Apex Soccer LLC activities under Soccer, Futsal or Goalkeeper Program offerings.

I authorize the Apex Soccer LLC staff in attendance at any Apex Soccer LLC activity to select and secure medical attention as may be necessary for my child as a result of injuries or other events requiring emergency care while I/we are not in attendance at such event.

I hereby release the Apex Soccer LLC staff from any and all liability that occurs during my son/daughter's participation in the athletic programs offered by Apex Soccer LLC.
